Online Availability of Florida Health Certificate for the Sale of a Dog or Cat

The Official Certificate of Veterinary Inspection (OCVI) for Intrastate Sale of a Dog or Cat (DACS Form 09085, Rev. 07/08), is available to Veterinarians at no cost as a downloadable form from the Division of Animal Industry’s website. The new form may be copied or reproduced, as long as it is not altered or modified in format or content. In addition, practice management software vendors may also incorporate the form into their applications, as long as the certificates produced are identical to the currently approved DACS Form 09085
The Pet Law F.S. 828.29 and companion Rules (Chapters 5C-27 & 5C-28) require that the completed original OCVI be furnished to the Purchaser, one copy be retained by the Seller for one year after the sale of a dog or cat, and a copy retained by the examining veterinarian for one year after the examination date. (Note that the Department of Business and Professional Regulation/Board of Veterinary Medicine considers the certificate retained by the veterinarian as a medical record, and as such must be retained for a period of 3 years.)
Previous hard copy versions issued by the Department of Agriculture & Consumer Services, which are 3-part carbonless certificates sold in books of 25, can still be used until supplies are depleted. The newer version (Rev. 07/08) is available for purchase from the Division of Animal Industry for those practices that prefer the 3-part carbonless forms.
